This is to certify that I have examined P. of Blackfriars road, Salford & that with the aid of glasses his eyesight is good & has no affection of his limbs or joints or skin, has never had fits & has no incontinence of urine has no infections or contagious disease or scrofula has had measles & whooping cough, never had smallpox & has been vaccinated, of good health
